How to Apply

Accepting applications for PGY V. All incoming residents must satisfactorily complete a PMR residency in a ACGME accredited institution. Thus, we urge all interested applicants to apply for the fellowship at the end of their PGY III or in the beginning of the PGY IV senior year. All interested applicants must do the following before an interview will be granted:

Email a CV and personal statement to: PMRPainFellowship@mednet.ucla.edu
Send or fax the following documents:
1. A recent passport size photo
  2. Cover Letter and Statement of Purpose
  3. Curriculum Vitae (Please include all contact numbers
and e-mail)
  4. 3 letters of recommendation (One must be from your Program Director)
 
5. Relevant information:
  Copy of USMLE/NBOME step I, II, III Score Reports
  Copy of Medical School Dean’s Letter
  Copy of SAE-R Score Reports
  Copy of Medical School Transcripts and Diploma

California Medical License
  Diploma ACMGE PR&M program
  California Radiology Supervisor's License
