- From the 405 Freeway southbound, exit at Wilshire Boulevard (West), and proceed west to 16th Street, turn left, and we are on your left.

- From Pacific Coast Highway, southbound, take the California Incline to Ocean Avenue, right (south) to Wilshire Boulevard, left (east) to 16th Street, turn right (south), and we are on your left.

- Metro and Santa Monica Blue Bus Lines offer service to Wilshire and Santa Monica Boulevards near Santa Monica—UCLA Medical Center.

- There is valet parking at the hospital across the street. With validation, the maximum cost is $8,00. There is metered parking on the street. There is also parking available in the subterranean garage at 1245 16th Street at a maximum cost of $10.50. Please note the office does not validate for parking.

- Bring your films, any medical records that pertains to your current condition, and insurance card with you to your appointment.

- Have your insurance card and referring/PCP doctor(s) name, address, and telephone number available.

- If you have managed care insurance, you must bring an authorization or referral for consultation or be prepared to assume the financial responsibility at the time services are rendered by cash, check, or credit card. We cannot bill your HMO without prior authorization. This guideline applies both to initial consultations and follow-up appointments beyond the 90-day postoperative period.
